


We sincerely welcome you as our patient and are pleased that you have chosen Southwestern Medical Center. We are committed to making your hospital visit as comfortable and convenient as possible from beginning to end. To help you know what to bring and where to go, we are glad to provide the following registration information for you.
Please bring any necessary insurance forms, insurance company identification cards or Medicare cards. Please report to the Admitting Office on the first floor of the hospital off of the main lobby, one hour before you are scheduled for surgery, or as your doctor instructs.
After completion of any laboratory, X-ray examinations or other tests that may be required, you will be escorted to the appropriate department.
Inpatient Registration Information
When You Arrive, please come to the Admitting Office on the first floor just off the main lobby. If you returned your pre-admission form before your admission day, your hospital admission record should be ready for you.
The Admitting Coordinator will then ask you some additional questions and will ask you to sign the required "Consent for Treatment" forms; as well as any insurance or Medicare forms that may be necessary.
Depending upon the financial arrangements you intend to make, you may also be asked to post a cash deposit.
Please be sure to bring your insurance company identification card or policy number, and/or Medicare identification card with you when you are admitted.
Once the necessary paperwork has been processed, a hospital staff member will take you to your room, or to the laboratory or X-ray department if you are scheduled for any diagnostic tests.

You will then be prepared for your surgery.
Following surgery, you will be taken to the recovery room until you awaken from you general anesthesia. You will then return to your room on the Short Stay Unit where you will remain until you are discharged.
